Blended Case Management (BCM)

Blended Case Management (BCM) serves to link adults and adolescents who are diagnosed with serious mental illness to services and resources in the community.

Blended Case Management (BCM) serves to link adults and adolescents who are diagnosed with serious mental illness to services and resources in the community. The overall goal of BCM services is to ensure that an individual is linked with the necessary resources and supports that will help them live as independently as possible while maintaining mental health stability.

Working with their Blended Case Manager, individuals identify short-term, attainable goals and develop a plan on how to achieve them. The Blended Case Manager assists the individual with securing resources and supports necessary to reach these goals.

A BCM case manager will meet with individuals on their case load regularly to monitor their progress toward meeting their goals.

Determining Eligibility

In order to qualify for BCM services in Cambria County, an individual must:

Reside in Cambria County

Be diagnosed with a Serious Mental Illness

  • Six or more days of psychiatric inpatient with the past 12 months.
  • Met standards for involuntary admission within the past 12 months.
  • Receiving or in need of mental health services and services from at least two additional human service agencies.
  • Missed at least three or more mental health service appointments or had two or more face to face encounters with crisis intervention with the past 12 months.

These services are provided at no cost to the individual and are provided regardless of insurance coverage.
