Early Intervention Service Coordination

The Early Intervention Program provides free developmental screenings and evaluations to all children age birth to three years old.

Services and supports can be provided to infants and toddlers who meet the eligibility criteria. Services and supports are provided free of charge and in the child’s natural environment.

We use the coaching model that is designed to help families promote their child’s development during typical everyday routines at home and in their community.

Process for Obtaining Services

Parent or guardian makes a referral to our EI Program by calling 814-535-8531 or 814-472-4400.

Referrals can also be made by pediatrician offices, hospital Neonatal Intensive Care Units or other child programs.

Our EI Coordinator assigns the referral to one of our Service Coordinators (SC). The SC will contact the family to complete an intake. The SC will schedule a developmental evaluation with parent consent to determine eligibility for EI services.

Infants and toddlers must have at least one of the following to be eligible for EI services:

  • A 25% delay in one or more areas of development including:
    • Cognitive
    • Communication
    • Physical
    • Self-Help
    • Social/Emotional
  • An informed clinical opinion to determine that there is a delay even if it does not show up on the assessments.
  • A known physical or mental condition that has a high probability for developmental delays, such as Down Syndrome.

Determining Eligibility

Eligible

If eligible, we will work with you to develop outcomes for your child and to determine the types of services needed to address those outcomes.

Your Service Coordinator (SC) will work with you to develop an Individual Family Service Plan (IFSP) and you will choose which provider(s) you want to work on those outcomes with you and your child. The SC will make the referral and services will start within 14 days.

Not Eligible

If your child is not eligible for EI services, they may still qualify for Tracking services. This allows the SC to send you a developmental questionnaire every three months to monitor your child’s development. If concerns arise, your SC will discuss options available at that time.

If your child is not eligible for EI services or Tracking, you will be provided with recommendations for activities and resources to help you continue to promote good development.
